①법 제6조에 따른 퇴직급여금 지급을 받으려는 사람(이하 "신청인"이라 한다)은 별지 제1호서식의 퇴직급여금 지급 신청서에 다음 각 호의 서류를 첨부하여 위원회 또는 각군 본부에 제출하여야 한다. <개정 2006.6.12, 2010.11.2, 2019.3.25, 2024.7.30>
