장애인복지법 시행규칙 제68조(서식)

①제4조제1항에 따른 장애인등록카드, 제24조제1항에 따른 학비지급신청서, 제24조제1항 및 제38조제1항에 따른 소득ㆍ재산신고서 및 금융정보등의 제공 동의서, 제31조에 따른 자금대여신청서, 제32조제1항에 따른 자금대여 결정통지서 및 자금대여 관리카드, 제38조제1항에 따른 장애수당등 지급신청서, 영 제34조제1항에 따른 자녀교육비 및 장애수당등의 환수결정 통지서, 제44조의2제1항에 따른 장애인 거주시설 이용신청서 및 소득ㆍ재산신고서, 같은 조 제2항에 따른 장애인 거주시설 이용 적격성 및 본인부담금 결정 통보서, 제44조의7에 따른 현장조사서는 사회보장급여와 관련하여 보건복지부장관이 정하여 고시하는 공통 서식에 따른다. <개정 2010.3.19, 2012.4.10, 2012.7.27, 2016.5.25, 2016.11.24, 2017.8.9, 2022.9.6, 2024.2.8>
