Enforcement Rule of the Welfare of Older Persons Act, Article 29-9 (Issuance and Reissuance of Care Worker Certificates)

① 요양보호사 자격증을 발급 받으려는 사람은 별지 제20호의3서식의 요양보호사 자격증 발급 신청서(전자문서로 된 신청서를 포함한다)에 다음 각 호의 서류(전자문서를 포함한다)를 첨부하여 국시원에 제출해야 한다. <개정 2015.1.16, 2022.11.22>

② 국시원은 자격시험에 합격한 사람이 제1항에 따른 서류를 제출한 날부터 30일 이내에 별지 제20호의6서식의 자격증을 발급하고, 자격증 발급 사실을 그 합격자가 수료한 요양보호사교육기관을 관할하는 시ㆍ도지사에게 통보해야 한다. 이 경우 자격증 발급 사실을 통보받은 시ㆍ도지사는 별지 제20호의7서식의 자격증 발급대장에 그 발급사실을 기재해야 한다. <개정 2022.11.22>

③ 제2항에 따라 자격증을 발급받은 사람은 그 자격증을 분실 또는 훼손하거나 그 자격증의 기재사항 변경 등의 사유로 이를 재발급 받으려는 경우에는 별지 제20호의3서식의 요양보호사 자격증 재발급 신청서(전자문서로 된 신청서를 포함한다)에 다음 각 호의 서류(전자문서를 포함한다)를 첨부하여 본인이 수료한 요양보호사교육기관을 관할하는 시ㆍ도지사에게 제출해야 한다. <개정 2011.4.15, 2015.1.16, 2022.11.22>
