고용보험법 시행규칙 제5조(피보험자격의 취득ㆍ상실신고 등)

① 영 제7조제1항 전단 및 같은 조 제2항에 따라 사업주나 하수급인이 근로자의 피보험자격의 취득 또는 상실에 관한 사항을 신고하려면 다음 각 호의 구분에 따른 신고서를 근로복지공단에 제출해야 한다. <개정 2008.4.30, 2016.11.17, 2020.12.10, 2021.7.1>
