고용보험 및 산업재해보상보험의 보험료징수 등에 관한 법률 시행규칙 제16조의9(휴직ㆍ전보 등의 신고)

① 영 제19조의7제7항제1호부터 제3호까지의 규정에 따라 근로자가 휴업ㆍ휴직, 출산전후휴가, 유산ㆍ사산 휴가, 육아휴직 또는 육아기 근로시간 단축으로 근로를 제공하지 않게 된 경우에 사업주는 별지 제22호의8서식의 근로자 휴직 등 신고서를 공단에 제출해야 한다. <개정 2015.12.31, 2020.12.10>

② 영 제19조의7제7항제4호에 따라 근로자가 다른 사업장으로 전보된 경우에 사업주는 별지 제22호의9서식의 근로자 전보신고서를 공단에 제출해야 한다. <개정 2020.12.10>

③ 영 제19조의7제7항제5호에 따라 근로자ㆍ예술인ㆍ노무제공자의 성명이나 주민등록번호 또는 같은 항 제6호에 따라 휴직 종료일이나 휴업 등 종료일이 변경된 경우에 사업주는 별지 제22호의10서식 또는 별지 제22호의17서식의 내용 변경 신고서를 공단에 제출해야 한다. <개정 2021.7.1>

④ 영 제19조의7제7항제7호에 따라 예술인 또는 노무제공자가 출산 또는 유산ㆍ사산 등을 이유로 노무를 제공할 수 없어 소득이 발생하지 않은 경우 사업주는 별지 제22호의18서식의 예술인ㆍ노무제공자 휴업 등 신고서를 공단에 제출해야 한다. <신설 2021.7.1>
