포항지진의 진상조사 및 피해구제 등을 위한 특별법 시행령 제15조의2(재심의)

① 법 제16조의2제1항에 따라 재심의를 신청하려는 자는 별지 제5호서식의 피해자 인정 및 지원금 지급 재심의 신청서에 재심의 신청 사유를 증명할 수 있는 서류를 첨부하여 심의위원회에 제출해야 한다.

② 법 제16조의2제2항에 따라 재심의 결정을 한 심의위원회는 다음 각 호의 사항을 기재한 별지 제6호서식의 피해자 인정 및 지원금 지급 재심의 결정서를 작성해야 한다.

③ 제2항의 결정서를 작성한 심의위원회는 지체 없이 별지 제7호서식의 피해자 인정 및 지원금 지급 재심의 결정통지서에 재심의 결정서 정본을 첨부하여 신청인(대리인을 선임한 경우에는 대리인을 말한다)에게 송달해야 한다.
