품종보호에 관한 절차를 밟은 자가 성명ㆍ 주소(법인의 경우에는 그 명칭, 대표자의 성명 및 영업소의 소재지를 말한다)를 변경하거나 정정하려는 경우에는 별지 제4호서식의 신고서에 다음 각 호의 서류를 첨부하여 산림청장ㆍ국립종자원장ㆍ국립수산과학원장 또는 심판위원회위원장에게 제출하여야 한다.
