법 제72조에 따라 감사를 청구하려는 자는 청구인의 성명ㆍ전화번호ㆍ생년월일ㆍ주소 및 직업을 기재하고 서명 또는 날인한 문서(이하 "감사청구서"라 한다)로 하되, 그 중 5명 이내의 대표자를 선정하여 감사청구서에 기재하여야 한다. <개정 2013.1.16>
