국민건강보험법 시행규칙 제57조(소득 축소ㆍ탈루 자료 송부의 서식 등)

공단이 법 제95조제1항에 따라 국세청장에게 소득 축소ㆍ탈루 자료를 송부할 때에는 별지 제38호서식의 소득 축소ㆍ탈루 혐의자료 통보서에 따른다. 이 경우 사용자나 세대주가 공단에 신고하거나 제출한 보수 또는 소득에 관한 자료와 공단이 조사한 증명자료를 첨부하여야 한다.
